This form must be completed if you are paying
for your trip with a credit card.

Visa Information
American citizens (and many other citizens) require
a visa to enter Brazil. This link provides information
on obtaining your visa. You can check requirements
and download an application from the Brazilian
Consulate.
Center
For Disease Control (CDC)
No medical precautions or shots are required of
travelers entering Brazil from North America,
but depending on where you are going and what
you are doing in Brazil, you should check with
the CDC or you local doctor about some precautions
you may want to consider.
